1. I understand that although massage therapy can be very therapeutic, relaxing and reduce muscular tension, it is not a substitute for medical examination, diagnosis and treatment.

2. Massage should be be done under certain medical conditions. I affirm that I have answered all questions pertaining to medical conditions truthfully. I also agree to update any medical conditions, if needed, an there shall be no liability on Knead Professional Massage.

3. This is a therapeutic massage establishment and any sexual remarks or advance will result in termination of the session and I will be liable for payment of the scheduled treatment.

4. I agree to hold Knead Professional Massage harmless in the event that I contract the COVID-19 virus.

 Health & safety measures are seriously taken here. Mask is required. Termperature check is required. Staff wear masks. Staff get temperature checks. Staff required to disinfect surfaces between visits.
